Travel order 3. Previous Medical cert, if there is any 4. Chest X-ray (within 3 months) 5. CBC (within 3 days) **Non-LSI** 1. Non LSI Certification 2. Recent Chest X-ray (within one month) preferably X-ray results with a photo of the client 3. 1×1 recent photo (White Background) **Non-LSI Renewal Med cert requirements** 1. Updated Non LSI Certification within 7 days of issuance 2. Chest X-ray (within 3 months) 3. CBC (within 3 days) 4. Previous Medical certificate 5. Company ID/Cert of employment **OFW** 1. IF approved, an online appointment and priority number will be given 3. During the scheduled time and date – present the original copies of the requirements for assessment ❇️ We encourage the public to avail this process to decongest the crowds that may pose a safety and health risk during these times.
